Organization
Islamic Center for Research and Business Incubator Inc. (ICRBI) is developing a nonprofit senior connected care model in California — helping older adults and families become ready for telehealth, RPM, chronic-care support, and community-based health navigation.
Core idea
Not another device company or RPM software platform. ICRBI is the trusted community layer connecting seniors, families, faith/community sites, clinical partners, and technology vendors into a practical care-readiness model.
The problem
Seniors face device access, digital literacy, caregiver, trust, transportation, language, and navigation barriers that limit the value of telehealth and RPM even when the technology exists.
The pilot
A 6-month San Diego County pilot for roughly 50–150 seniors across two faith/community sites, focused on hypertension, diabetes, heart health, respiratory risk, fall risk, and social isolation.
Partner roles
Licensed clinical partners manage medical decisions, treatment, escalation, and any clinical RPM billing. Community sites build trust and access; vendors support devices; payers advise on care gaps and reporting.
Sustainability
A blended model exploring Medi-Cal CBO/CHW pathways, managed care contracts, clinic/FQHC partnerships, grants, sponsorships, vendor pilots, and donations — planned pathways, not current approvals.
